To set up new service in Harris County M.U.D. No. 208, the following items must be completed before an account can be set up:

Payment of Deposit The deposit amount is $50.00 Please note: This deposit may be refunded after a two-year period of prompt payments upon written request. Deposit checks should be made payable to Harris County M.U.D. No. 208
and sent to the following address: Payment may also be made in person at H2O Consulting, Inc., located at 5870 Highway 6 North, Suite 215. |
| 2 | A Completed Service Agreement. |
| 3 | An application must be filled out. An application fee of $35.00 is also required prior to service being set up. |
TOTAL AMOUNT REQUIRED, PRIOR TO SERVICE BEING SET UP IS $85.00 PAYABLE TO HARRIS COUNTY M.U.D. NO. 208.
